Frequently Asked Questions

You have questions. We have answers. Below are some of the common questions we frequently get asked. Click on a question to reveal the answer.
Where do I fill out paperwork?

Visit our new patient’s page to find the associated forms. Print and complete the form and bring it with you to your first visit.

What do I do if I need to cancel or reschedule my appointment?

Please give us a 24-hour notice if you are unable to attend a scheduled appointment so the appointment time can be available to another patient. However, we understand unexpected life events happen. If a sudden change occurs, please call and let us know as soon as possible.

Do I see the same therapist at every appointment?

We try our best to keep you with the same therapist throughout your treatment. On a rare occasion, you may see another therapist if yours happens to take an unexpected leave. We will always communicate ahead of time if you won’t see your regular therapist.

Are there private treatment rooms?

All of our locations have private treatment rooms.

What do I bring to my first appointment?

Arrive 10-15 minutes before your appointment with your completed paperwork. Bring your insurance information (i.e., insurance card, workman’s compensation claim information, or auto insurance) and your driver’s license or a valid ID. Any recent, related test results, such as MRI or X-Rays, may be helpful as well.

Do I need to see my doctor before going to physical therapy?

A doctor’s referral is not needed to access physical therapy in Nebraska.  Each insurance has different requirements however.  It's important to contact you insurance to understand aspects of your plan.  Insurance plans have different authorization requirements, co-pays, co-insurance and allowed visits.  Needs some help with this?  We're always glad to help answer questions or point you in the right direction to find a solution.
